Abstract
Le Syndrome de Leigh(SL) est une mitochondriopathie rare de l’enfant caractérisée par un polymorphisme clinique. La maladie apparait généralement avant l’âge de 2 ans. Dans cette observation, nous rapportons une forme juvénile de SL. Il s’agit d’une patiente âgée de 13 ans, issue d’un mariage consanguin, qui a présenté une baisse importante de l’acuité visuelle à l’âge de 9 ans. A 13 ans, elle a présenté des troubles bulbaires et de la marche. L’examen neurologique a retrouvé un syndrome pyramidal, un syndrome extrapyramidal, un syndrome neurogéne et une cyphoscoliose. L’IRM cérébrale a objectivé un hyper signal bilatéral et symétrique au niveau du tronc cérébral, des noyaux gris centraux et de la région périaqueducale. La spectro-IRM cérébrale a retrouvé un pic de lactates.
